What Is Geriatric Care Management?

Care management is a holistic, professional service that plans and coordinates care for older adults to maximize their independence, health, and safety. Geriatric care management also provides critical support for family caregivers coordinating care for aging parents or loved ones, helping families navigate complex medical needs and healthcare systems.

As an Licensed Clinical Social Worker (LCSW) and Certified Dementia Care Specialist (CDCS), I act as a liaison, providing services like, comprehensive assessments, crisis intervention, and navigate complex medical/legal systems for families.

Service begins with a comprehensive assessment and individualized care plan, followed by ongoing care coordination, medical advocacy, assistance with electronic health records, and clinical oversight as needs evolve.

Care management complements - but does not replace - medical or home health services.
